Registration Information

BRING INSURANCE CARDS, AND PICTURE I.D. (such as driver's license) when you come to the Surgery Center.

FOR PEDIATRIC PATIENTS, please make sure you have their social security number.

IF YOU HAVE A LIVING WILL we would like a copy for our records if possible.

IF YOU HAVE POWER OF ATTORNEY OR GUARDIANSHIP of the patient, please bring legal documentation of such.

THERE WILL BE NUMEROUS FORMS to sign and to be filled out that pertain to your state of health and personal information.

TO PROTECT YOUR PRIVACY the only people that will be allowed in the treatment area are the ones you list on the consent form - which you will fill out in the registration area. Only two people will be allowed in the treatment area at a time.



Prostate Cancer Awareness Month

Prostate cancer is the second leading cause of cancer death among men. Those men at highest risk are African-American men and those with a family history of prostate cancer diagnosed under the age of 60. The American Cancer Society recommends that all men should get a PSA blood test and digital rectal exam beginning at age 50 except those at high risk, who should start at age 45.
